Getting your card,the next step

  • Your Simply Healthcare Provider will submit your eligibility to Illinois Dept of Public Health/Medical Cannabis Patient Registry so that you can apply for your card
  • You will need to set up or log into your ILOGIN to apply for your card. There is a fee associated with the application and you can pay for a one, two or three year card.The fees are reduced to 50% for veterans and hospice patients are not charged.
  • Call our care team at if we can help you navigate the application process online. Your Simply Healthcare Provider is available for follow up calls any time after your first appointment. Please call with any questions or to discuss effective dosage or a possible recommendation for increased limits. There is no charge for your first follow up call. (recertifications don’t qualify as follow ups)
